INCIDENT REPORT FORM
In the event of an incident or situation within a team, at a game or during training, we would encourage you to discuss this directly with the coach/player/parent involved initially.
If the situation is not resolved, please contact the age group coordinator to ask for assistance.
If the situation is still unable to be resolved, you may complete and submit the Incident Report Form.
This form is reviewed by two independent committee members who will then investigate the situation and assist in resolving it. It is a completely transparent and independent process, one that we have spent much time developing to ensure all parties are treated fairly and hopefully come out happy!
